Does the suspension actually work or is it just decorative?

It legit works. Each wheel has a spring shock absorber that compresses and extends over bumps. You can actually see them move when the ATV hits uneven ground. Makes a huge difference on grass, gravel, and rough terrain compared to rigid wheels.

Can this handle actual off-road terrain like dirt and grass?

Yeah, that's what it's built for. The wear-resistant tires and dual motors give it enough grip and power to handle grass, packed dirt, gravel, brick paths, beach sand, and asphalt. Won't conquer massive hills or thick mud, but typical backyard terrain is no problem.

How fast does it actually go and can I limit the speed?

Max speed is around 5mph on high setting, which feels fast for a kid but is safe. Low setting caps it around 2-3mph. Most parents start kids on low speed until they get comfortable with steering and throttle control, then bump it up to high when they're ready.

My kid is 7 and kind of big—will they fit or is this just for toddlers?

This ATV is the real deal. My 6 year old takes it all over our property—grass, dirt paths, gravel driveway, you name it. The suspension actually works and you can see it compress when he hits bumps.
